Parent/Legal guardian must accompany the student. Students 18 and over can receive services without parents/guardians.
Bring current immunization record
Must bring Medi-Cal card if your child has Medi-Cal insurance card

Sports Physicals: Please wear tennis shoes to run.

PRIVATE INSURED STUDENTS, please call your healthcare provider for immunizations or you can call CVS, Rite-Aid, Walgreens or Walmart pharmacies to see if they take your insurance and offer the immunizations your child needs.
